We present a special case of a HIV-positive patient with a complex extrasphincteric anal fistula who was treated successfully with fibrin glue application. Anorectal procedures (for anal condylomata, fistula in ano, hemorrhoids, and perirectal abscess) are the most common operations performed in HIV-patients. The main objective in the treatment of perianal fistula is the healing of the fistula by closing the internal opening while preserving the anal continence.
